How to Manage Your Family's Vaccination Records

  1. GATHER

    To begin, it is important to gather as many records as can be recovered. Check with these sources, recommended by the Immunization Action Coalition:

    a. All previous health care providers
    b. Personal papers, such as baby books and camp medical history forms
    c. Schools, including college admission offices for adult children
    d. Job or military records

    If you are still unable to locate records of required vaccines, check with your state's Immunization Information System (IIS).
